Our Services

Our physicians specialize in the treatment of genito-urinary cancers (kidney, adrenal, bladder, prostate, penile and testicular), comprehensive treatment of urinary stones, diseases of the bladder and prostate (including urinary tract infections, prostate enlargement and prostatitis), urinary incontinence, male infertility and erectile dysfunction. All physicians at Eastside Urology Associates are fully trained in adult and pediatric urology. We perform vasectomy, vasectomy reversal, adult and pediatric circumcisions, hernia and hydrocele repairs, sling incontinence procedures and repair of cystoceles.

Hours

Clinic hours are 8am to 5pm Monday through Friday. If you have a urological emergency during the evening or on a weekend, call our main number (425) 899-5800 to have the answering service contact the urologist on-call.

What To Bring To Your Appointment

• Insurance card
• Completed registration paperwork
• Referral, if required by your insurance
• Pertinent chart notes/test results/films from your primary physician or previous urology provider
• List of current medications, with dose and frequency and any drug allergies

Insurance and Payment Information

• Eastside Urology Associates is a provider for Medicare and most major insurance plans.
• Anything not covered by insurance will be your responsibility.
• We request payment at the time of your appointment for services that are not covered by insurance.
• Your insurance company may also require you to pay a co-payment at the time of your appointment.
• If you have a managed care plan, you are responsible for making sure your Primary Care Provider (PCP) sends us a referral prior to your appointment.
• Questions regarding your coverage and benefits should be directed to your employer or insurance company.
• Regardless of your insurance coverage, you are responsible for the timely resolution of your account balance. When necessary, our staff will work closely with patients who require a payment plan.
• We accept cash, check, MasterCard or VISA.
• A $20 processing fee will be charged for checks returned by the bank.

We strive to process payments accurately. If you have a question or find an error, please call our bookkeeper as soon as possible.

Office Policies

• Please call your pharmacist for refill requests and allow 48 hours.
• Medical records may be copied with a signed release from you and 72 hours notice.
• We request 24 hours notice of appointment cancellation.
